Add a Verification Note to Every AI-Assisted Decision

Polished output can hide unfinished checking

AI-assisted summaries, drafts and comparisons can look complete even when an important detail has not been confirmed. A team member who receives the document later may not know which statements came from source material, which were interpreted and which still need review.

The answer is not a long technical report. Add a short verification note wherever the work will be stored or shared.

Record five useful details

Before an AI-assisted document supports a business decision, record:

  • The question: What decision or task was the work meant to support?

  • The sources: Which documents, links or approved internal records were used?

  • The human checks: Which dates, amounts, names, requirements or quotations were compared with the originals?

  • The uncertainty: What remains incomplete, disputed or dependent on another person?

  • The reviewer: Who checked the work, and when?

This note can sit at the bottom of a working document, in the project file or in a decision log. It does not need to be public. Its purpose is to help the next person understand what can be relied upon and what still requires judgment.

Keep the original material close

Save or link the source documents beside the AI-assisted output. Do not make the summary the only record. If the interpretation is questioned later, the reviewer should be able to return to the original information without reconstructing the entire process.

The person responsible for the decision should still read the critical evidence. A verification note supports accountability; it does not transfer responsibility to the tool.

A hypothetical example

Imagine a procurement coordinator uses AI to compare three vendor proposals. Before sharing the comparison, the coordinator checks each quoted price, delivery date and renewal term against the original proposals.

The note identifies the three source files, records that the commercial terms were manually checked and flags one vendor’s support response time as unconfirmed. The manager can now review the comparison while seeing exactly where one follow-up question remains.

Try it today

Choose one AI-assisted document your team is using. Add a five-line verification note with the question, sources, checks, uncertainty and reviewer. If any line cannot be completed, pause before treating the document as decision-ready.
